Our patient list has grown very rapidly over the last 3 years and, in order to keep it to a safe and manageable level, the GP Partners have taken the difficult decision to restrict the number of new patients who may join our Practice.

We are currently operating a waiting list for places and we register a limited number of patients from the waiting list on the first working day of each month.

If you live within our Practice boundary and qualify for NHS treatment, you are welcome to apply for a place on our patient list.
The Practice boundary is:

Queensferry Road to the north; Drumbrae, Meadowplace and Broomhouse Roads to the west; Calder Road, Longstone Street, Inglis Green Road and Slateford Road near Ardmillan, left along Murieston Road, continuing alongside the railway line to Haymarket, bypassing Dalry.

Please be aware that we are unable to add your application to our waiting list unless you have changed your address and, as a result, now live within our Practice boundary area.

We will continue to register babies born to existing patients, and immediate family members who come to live permanently at the address of existing patients. We can also treat temporary residents who are living within our Practice boundary for up to 3 months.

To join our waiting list:

Please complete the NHS Scotland 'Application to Register with a General Medical Practice', which can be found at:

You may then apply to register in one of two ways:

(i)             Apply by email to, 'Application to Register' in the subject line. Your completed application form must be accompanied by copies of supporting documents*. Receipt of your application will be acknowledged and you will be added to our waiting list. We will contact you after the first working day of the following month to advise whether or not we have been able to register you

(ii)            While electronic applications are preferred, you may wish to bring the completed application form and supporting documents* to the surgery in person, between 0930-1730, Monday to Friday (note that we are closed on Thursdays 1215-1315). On receipt of your application, you will be added to our waiting list and informed after the first working day of the following month whether or not we have been able to register you

If we have been unable to offer you a place in the month after you have provided us with a completed application, you can stay on our waiting list if you wish and we will contact you again the following month. Our waiting list is lengthy and we can accept only a small number of applications each month. You are advised to consider contacting other local Practices who may be able to accept your application more quickly.

*Supporting documents: Together with your completed application form, we request photographic proof of identity (for example: passport or photographic driving licence), together with a recent document which shows your name and address. Acceptable documents are a landline telephone or utility bill, or a Council Tax bill; all must be dated within the last 3 months. Where applicable, we also require sight of your current visa or residency permit.. Please bring original documents if you are applying to the Practice in person or, if applying by email, scanned copies, or clear photographs taken on a mobile phone, are acceptable.

When we are able to offer you a place on our patient list, you will be asked to complete and submit a New Patient Health Check questionnaire prior to your first appointment.
